The first quest update can take a long time to get for some players, luck of the draw. I don't know whether or not you need to be in the gated area to get the quest update but that is where I obtained it after 2 hours of hunting outside the gate. You will also gain access to Ghoul Lord Yesoth upon completing Part 1.
- Reward: Forged Skull Shield, Access to Ghoul Lord Yesoth
- Part 1: Travel towards Farstead Keep and then head East bellow the river to Gnobol Manor. Speak with Adelia Gnobol, after a lengthy story she wants you to find her husband and children down by the Mausoleum. Head straight South to the cemetery and kill Ghouls until you get a quest update that you've found a Jewel. It shouldn't matter which type of Ghoul you kill.
- Part 2: After you receive the quest update head towards the Southwest corner. There is a small building with a portal inside, step into the portal. If you try to go into the portal before you obtain the Jewel it will tell you that you must find the Jewel. You don't need to do anything down there just go back up the stairs and you will find yourself in the building above the portal. Click on the bones that lays in the corner for a quest update and receive Otto Gnobol's Diary. Click on the book and read his tale, the last couple pages gives a clue on how to end the quest. To get out you must click on the switch on the wall.
- Part 3: Return to Adelia Gnobol and deliver the bad news. If you do not have the book in your backpack, or if it is in a container she will tell you to return when you have the book. You will not receive a quest update when you are done speaking to her but will effectively end the quest. If you try to speak to her she will not remember who you are. What you need to do next, as the diary hinted at, is to head towards the Southwest corner of the house and click on the harp. Again you must have the Jewel before being able to do this part. Enter the secret room that just opened up and click on the right bookcase for your reward. Receive Forged Skull Shield. To get out you must click on the torch to reopen the secret door.
